`stacks` `threads` & `unwind` are commands related to kernel
thread, group them one level inside a main `thread` command,
and rename `threads` to `list`:
- `kernel threads` -> `kernel thread list`
- `kernel stacks` -> `kernel thread stacks`
- `kernel unwind` -> `kernel thread unwind`
Additionally, rename and mark the `thread_valid()` function
as `__maybe_unused` as it might be unused for architectures
without stackwalk implementation, and use the locked version
of the thread iterator (`k_thread_foreach()`)

This commit introduces a new ARCH_STACKWALK Kconfig which
determines if the `arch_stack_walk()` is available should the
arch supports it.
Starting from RISCV, this will be able to converge the exception
stack trace implementation & stack walking features. Existing
exception stack trace implementation will be updated later.
Eventually we will end up with the following:
1. If an arch implements `arch_stack_walk()`
`ARCH_HAS_STACKWALK` should be selected.
2. If the above is enabled, `ARCH_SUPPORTS_STACKWALK` indicates
if the dependencies are met for arch to enable stack walking.
This Kconfig replaces `<arch>_EXCEPTION_STACK_TRACE`
2. If the above is enabled, then, `ARCH_STACKWALK` determines
if `arch_stack_walk()` should be compiled.
3. `EXCEPTION_STACK_TRACE` should build on top of the
`ARCH_STACKWALK`, stack traces will be printed when it
is enabled.
4. `ARCH_STACKWALK_MAX_FRAMES` will be removed as it is
replaced by `ARCH_STACKWALK_MAX_FRAMES`

Namespaced the generated headers with `zephyr` to prevent
potential conflict with other headers.
Introduce a temporary Kconfig `LEGACY_GENERATED_INCLUDE_PATH`
that is enabled by default. This allows the developers to
continue the use of the old include paths for the time being
until it is deprecated and eventually removed. The Kconfig will
generate a build-time warning message, similar to the
`CONFIG_TIMER_RANDOM_GENERATOR`.
Updated the includes path of in-tree sources accordingly.
Most of the changes here are scripted, check the PR for more
info.

Simple rename to align the kernel naming scheme. This is being
used throughout the tree, especially in the architecture code.
As this is not a private API internal to kernel, prefix it
appropriately with K_.

Include the full version in the output of the "kernel version" shell
subcommand. Previously, EXTRAVERSION was not included, causing
e.g. "3.6.0-rc1" to be printed as "3.6.0".

Always use k_thread_foreach_unlocked with callbacks which print
something out to the shell, as they might call arch_irq_unlock.

There are several subsystems and boards which require a relatively large
system heap (used by k_malloc()) to function properly. This became even
more notable with the recent introduction of the ACPICA library, which
causes ACPI-using boards to require a system heap of up to several
megabytes in size.
Until now, subsystems and boards have tried to solve this by having
Kconfig overlays which modify the default value of HEAP_MEM_POOL_SIZE.
This works ok, except when applications start explicitly setting values
in their prj.conf files:

The vast majority of values set by current sample or test applications
is much too small for subsystems like ACPI, which results in the
application not being able to run on such boards.
To solve this situation, we introduce support for subsystems to specify
their own custom system heap size requirement. Subsystems do
this by defining Kconfig options with the prefix HEAP_MEM_POOL_ADD_SIZE_.
The final value of the system heap is the sum of the custom
minimum requirements, or the value existing HEAP_MEM_POOL_SIZE option,
whichever is greater.
We also introduce a new HEAP_MEM_POOL_IGNORE_MIN Kconfig option which
applications can use to force a lower value than what subsystems have
specficied, however this behavior is disabled by default.
Whenever the minimum is greater than the requested value a CMake warning
will be issued in the build output.
This patch ends up modifying several places outside of kernel code,
since the presence of the system heap is no longer detected using a
non-zero CONFIG_HEAP_MEM_POOL_SIZE value, rather it's now detected using
a new K_HEAP_MEM_POOL_SIZE value that's evaluated at build.

MISRA Rule 5.7 requires uniqueness of tag identifiers. Shell is
frequently problematic because many code uses `const struct shell
*shell`. This causes CI noise every time one of these shell files is
edited, so let's update all of them with `const struct shell *sh`
instead.

We do the call outside of the for loop so that it only happens once,
rather than on every iteration.

Also enable it for the Bluetooth Shell.
Then one can compile-in a lot of BT modules like so:

When threads are in more than one state at a time, k_thread_state_str()
returns a string that lists each of its states delimited by a '+'.
This in turn necessitates a change to the API that includes both a
pointer to the buffer to use for the string and the size of the buffer.

Allow time for the shell to successfully echo the reboot command input
before the reboot abruptly terminates it. n This can help external
systems that interact with the shell and require the reboot command's
echo to successfully complete to synchronise with the device.

Remove this intrusive tracing feature in favor of the new object tracing
using the main tracing feature in zephyr. See #33603 for the new tracing
coverage for all objects.
This will allow for support in more tools and less reliance on GDB for
tracing objects.

The clock/timer APIs are not application facing APIs, however, similar
to arch_ and a few other APIs they are available to implement drivers
and add support for new hardware and are documented and available to be
used outside of the clock/kernel subsystems.
Remove the leading z_ and provide them as clock_* APIs for someone
writing a new timer driver to use.

Sometimes it is hard to tell which instance of a thread is which
in the printed list, based solely on the name (if present) and
the k_thread pointer, so also print the thread entry fn pointer.

When calculating the size of unused interrupt stacks on SMP,
the "unused" variable is not cleared between CPUs. So this
value keeps incrementing and does not reflect actual unused
size for CPUs other than the first one. So clear the "unused"
variable for each CPU.

The set of interrupt stacks is now expressed as an array. We
also define the idle threads and their associated stacks this
way. This allows for iteration in cases where we have multiple
CPUs.
There is now a centralized declaration in kernel_internal.h.
On uniprocessor systems, z_interrupt_stacks has one element
and can be used in the same way as _interrupt_stack.
The IRQ stack for CPU 0 is now set in init.c instead of in
arch code.
The extern definition of the main thread stack is now removed,
this doesn't need to be in a header.

The existing stack_analyze APIs had some problems:
1. Not properly namespaced
2. Accepted the stack object as a parameter, yet the stack object
does not contain the necessary information to get the associated
buffer region, the thread object is needed for this
3. Caused a crash on certain platforms that do not allow inspection
of unused stack space for the currently running thread
4. No user mode access
5. Separately passed in thread name
We deprecate these functions and add a new API
k_thread_stack_space_get() which addresses all of these issues.
A helper API log_stack_usage() also added which resembles
STACK_ANALYZE() in functionality.

The current code is assuming that the pointer to the thread structure is
32bit, casting it to u32_t before printing its address. This is wrong on
64bit architectures (CONFIG_64BIT) and the compiler complains.
Fix the problem by using '%p' to print the address.

It is planned to deprecate SHELL_CREATE_STATIC_SUBCMD_SET macro
which is replaced by SHELL_STATIC_SUBCMD_SET_CREATE.
Additionally, removed irrelevant comments about alphabetical
ordering which is no longer needed.
