The kernel configuration option SHARED_IRQ_1_PRI is dependent upon
SHARED_IRQ_1, not SHARED_IRQ_0.

Changes the default IRQ priority level from 0 to 2 for the following
kernel configuration options as priorities 0 and 1 are reserved for the
first 32 IDT entries.

Removes permalinks and sets the correct version and release. The change places
the copyright notice and license in the footer of every page.
Both copyright and license do not appear in the website's version.

Renames the Kconfig option NO_NESTED_INTERRUPTS to NESTED_INTERRUPTS
as it is typically easier to follow positive logic than it is to
follow negative logic.

This patch corrects gpio_dw_setup to use the MMIO base address
determined by PCI enumeration when that enumeration feature is enabled
instead of always using the default base address specified at compile
time.

To make the documentation readable from the source I want to get rid of the
substitutions for the project name an code name. This does not add any values
and makes it unreadable when looking at the text files directly. It also causes
some issues when people use those without actually knowing what they represent,
resulting in some weird and redundant language.
